Logging into the Form Health app

Are you having trouble logging in to the Form Health app? Follow the steps below for assistance!

1. If you haven't already downloaded the Form Health app, you'll want to search for it in your phone's app store. Search "Form Health" and look for this logo:

2. When you enrolled in Form's program, you provided an email address and phone number. You can log in using either, however, a phone number is often quicker and more convenient.

   
   

3. Once you enter the code, click "verify" (if the app doesn't refresh on its own), and you'll be logged in!

A few additional tips:

1. You can also use the Form app on a tablet, like an iPad or Android tablet!
2. We have a MyForm portal where you can log in online. Here is the link to log in: https://my.formhealth.io/login/phone. Please note that not all features on the mobile app are available in the MyForm portal at this time.
3. Requesting the login code by email can take up to 10 minutes. If you are requesting a code by email and don't receive it within 10 minutes, make sure to check your spam folder.
