Logging into the Form Health app
Are you having trouble logging in to the Form Health app or Web Portal? Follow the steps below for assistance!
Logging into the Form Health App
- If you haven't already downloaded the Form Health app, you'll need to download it onto your mobile phone. Click this link and then choose your phone's app store. You can also download it by searching "Form Health" in your phone's app store and look for this logo:

- When you enrolled in Form's program, you provided an email address and phone number. You can log in using either, however, a phone number is often quicker and more convenient.

3. Once you enter the code, click "verify" (if the app doesn't refresh on its own), and you'll be logged in!

Logging into the Form Health App
- Go to my.formhealth.io - this is our Form Health web portal

- Once you enter the code, click "verify", and you'll be logged in!

A few additional tips:
- You can also use the Form app on a tablet, like an iPad or Android tablet!
- Requesting the login code by email can take up to 10 minutes. If you are requesting a code by email and don't receive it within 10 minutes, make sure to check your spam folder.
- You may be asked if you want to enable biometric login (face ID or fingerprint scan). This is completely optional, but many patients find this more convenient than requesting a code each time. If you decide not to enable it now, you can always enable it later on in your phone's settings.
