Submitting a Food Log Entry
- On the Homepage of the Form app, you can submit a food log entry by scrolling to the bottom of the home page. You'll see a big red button that says "Add Food Log Entry"
- Once you click either button, you can choose a photo from your camera roll, take a photo in real-time, or return to the homepage if you change your mind.
- Please ensure your phone settings are set to give Form Health access to your camera roll. This will allow you to take pictures and choose photos from your camera roll. Scroll to the bottom of this page if you need help with this.
- You can also choose "Text Only" which will allow you to write a description of the meal, snack, or beverage you had without uploading a photo.
- Add comments (if you'd like) and submit to the food log!
- Once you click submit, you'll see a confirmation screen that your photo has been uploaded!
- You can view past food log entries by swiping left and right in the food log section of the homepage.
- To view a specific photo, simply click on a photo to enlarge it.
- Tap the photo again to show any comments you made.
- You can also scroll through one photo at a time in the food log by swiping to the left or right on the enlarged photo.
Checking Your Phone Settings for Camera Access
If you're unable to take a photo or take a picture from your camera roll when submitting a food log entry, you will want to check your phone's permissions to make sure you are allowing the FORM app access.
iPhone
- On your device, open the Settings app.
- Scroll down to the Form Health App.
- Click Form Health
- Click "Photos"
- Make sure either "Full Access" or "Limited Access" is checked off.
Android
- On your device, open the Settings app.
- Tap Apps.
- Tap the app you want to change. If you can't find it, tap See all apps. Then, choose your app.
- Tap Permissions.
- If you allowed or denied any permissions for the app, you’ll find them here.
- To change a permission setting, tap it, then choose Allow or Don't allow.
