Why did I receive an invoice for lab work my Form Health medical clinician ordered?

Please get in touch with your insurance provider directly to learn more about your laboratory coverage. Depending on your coverage limitations, you may need to have your labs ordered or drawn by your local PCP. Form Health is not responsible for any invoices or patient responsibility balances incurred from your chosen laboratory. If your insurance requires a copy of the lab order, please message your Care Coordinator in the Support channel in the Form Health app and they can email you a copy.
