Check Prescription Status

Prescriptions are typically sent within 24 hours of your visit. Please contact your pharmacy directly to confirm they have the prescription on file. Delays in processing by the pharmacy are commonly due to:

  1. The prescription requires prior authorization, which can take 7-10 days. Please contact your care coordinator in the "Support" channel of the Form app to check on the status of the prior authorization.
  2. The prescription is out of stock. In this case, it is recommended that you call other local pharmacies to see if they have your dose in stock. The new pharmacy can get your prescription transferred.

If your prescription has not been received by the pharmacy or there are other issues, please send a message to your Care Coordinator in the "Support" channel of the Form app. Please include all relevant details (e.g. the specific medication, pharmacy details).
