I need a refill in advance of my next appointment

You will be able to request a refill through the app for Form Health medications after you've had your first medical appointment.

If you have already met with your Form Health clinician and have an active prescription with Form Health, please check with your pharmacy first to confirm there are no remaining refills.

You can request a refill on the homepage of the Form Health app or in the Form Health web portal, my.formhealth.io.

Under "My Medications", you can request a refill by clicking the "Refill Medication" button, or by clicking "View All" to see your active Form Health prescriptions.

If your medication is not listed, you can select "Refill Other Medication" to enter the name, dosage, and frequency of the Form Health medication you'd like to refill. Once submitted, your clinician will be notified of your request for a refill.
