I need a refill in advance of my next appointment
You will be able to request a refill through the app for Form Health medications after you've had your first medical appointment.
If you have already met with your Form Health clinician and have an active prescription with Form Health, please check with your pharmacy first to confirm there are no remaining refills.
To request a refill of an existing medication or a new prescription, please navigate to the chat section of the Form Health app. Under "How can we help you?", select "Prescriptions" and then "I need a refill in advance of my next appointment." This will direct you to a form where you can select your medication name/dose, as well as your pharmacy and any side effects you're experiencing. Once submitted, your clinician will be notified of your request for a refill.
