In the MyForm portal (https://my.formhealth.io), navigate to the "Billing" page and click "Support".
If you don’t find your answer in the “Instant Answers”, you can type in the search field. If your answer still doesn’t come up, click “Get in Touch”. You will then be able to send our billing team a message. Please give our team 72 hours to answer your inquiry.
