I have new insurance information

You can update your insurance information in the app by following the steps below.

1. Open the Form app and navigate to the chat screen (click on the chat icon at the bottom of the app).
2. Click on "Account / Billing" then click on "I have updated insurance".

3. Select which insurance cards you'd like to upload.

   

4. Upload a photo of the front/back of your new insurance card(s) and click Submit.

   

5. Once submitted, we will update your insurance information in our system.
