I'd like to cancel or pause my account

You can cancel your participation in Form Health’s program at any time by completing this cancellation form or by messaging your Care Coordinator in the "Support" channel of the Form App. 

If you have already met with your Form Health medical clinician and/or dietitian, please keep the Form app on your phone for 2-3 business days in case your care team needs to get in touch with you to transition your care outside of Form Health. You are welcome to re-enroll at a later time by emailing us at help@formhealth.co to reinstate your account.
