I’ve enrolled in Form Health’s program, what should I expect now?

Welcome to Form Health! We are so excited to partner with you on your weight loss journey. 

Now that you’ve enrolled, check your email for a Welcome email from us. It will include a link to download the Form App. If you don’t see it, check your spam folder. Your email address may not be familiar with us yet and tag us a spam. 

Once you’ve downloaded the app, you’ll want to log in to complete your onboarding to-dos that you’ll find at the top of the homepage. Our enrollment team works behind the scenes to process your paperwork, collect your medical records from your primary care provider, and pair you with a care team that is licensed in your state and in-network with your insurance. Once we receive your medical records, you’ll be able to schedule with your care team! 

If there is some lead time to your first appointment, feel free to start tracking your meals using our food log feature, or review the resources shared with you in the app.
