When can I expect to receive the medication?

Every patient’s care at Form Health is personalized based on their weight, additional health conditions, lifestyle, and goals. You’ll discuss medication options and if an FDA-approved weight loss medication is right for you at your first appointment with your medical clinician. If a medication is prescribed, it will be part of a comprehensive, personalized plan that also addresses nutrition, physical activity, and mindset shifts. Any prescription is sent to the patient's pharmacy of choice to be filled.

Our team may need to submit a prior authorization to your insurance to request coverage. Although we do all we can to advocate for our patients, ultimately it is your insurance’s decision on whether they will cover a medication under your specific plan’s benefits. 

If you’d like to get a head start on determining which medications are covered under your benefits plan, please take a look at your insurance formulary or give them a call and ask about coverage. You may want to ask if it is covered and if it requires prior authorization. Some common FDA-approved weight loss medications include Wegovy®, Zepbound®, Saxenda®, and CONTRAVE®. Please keep in mind that even if a medication is covered, it is up to you and your medical clinician to determine if/which medication is right for you.
