Why do I owe an additional balance when I already pay a monthly membership fee?

You are currently on our Membership Plan which has a monthly membership fee. This fee covers access to the Form platform and covers your two monthly dietician visits. This fee is billed automatically to your payment method on file. This fee is FSA/ HSA eligible. However, we still bill your doctor's visits to your insurance. You are responsible for any co-pay deductible, or coinsurance associated with your insurance plan.
