Why does Form Health ask if I have had a recent hospitalization for a mental health condition?

If you have been in the hospital for a serious mental health condition, we need to be sure that your psychiatric care and condition is stable before you join Form Health, to ensure that participation in our clinic will be safe and effective. Ongoing serious mental health conditions can present barriers to receiving weight care, and starting weight loss medications during periods of active medication management for mental health may not be advisable. 

Our Form Health Care Advisors are available to discuss your individual situation. Patients who are receiving care from health care professionals for mental health conditions and are in stable clinical condition are very appropriate for, and likely to be helped by, our structured, supportive and compassionate approach to weight care.
