How do I Schedule an Appointment?

Appointments must be scheduled through the Form Health mobile app.


Scheduling Options on the Homepage

You can book your appointment using one of two options visible on the app's homepage:


A. The Quick Scheduling Banner

If you do not have an appointment scheduled, you may see a large orange banner at the top of the screen:

  • Action: Tap "Schedule Appointment" in the banner. *
    • Note: This banner only appears if there is immediate availability with the clinician you need.


B. The Appointments Section

You can also use the dedicated section lower on the homepage:

  • Action: Find your clinician's name (e.g., Manuel Martinez) and tap the "Schedule" button next to it.
