How do I join my appointments?

How to join your Form Health appointment from your phone or computer, and what to do if you have trouble.

When can I join?

You can join your appointment up to 5 minutes before the scheduled start time. If you tap the join link earlier than that, you'll see a screen letting you know it isn't time yet, along with your appointment details. The join button will become active automatically once the window opens.

How to join

  1. Open the appointment link from your text/email reminder, or tap Join Zoom in the Form Health app.
  2. Your appointment will open in a browser window and connect you to your care team automatically. If it doesn't, tap Join Now.
  3. Allow camera and microphone access if your browser or phone prompts you.

Prefer the Zoom app? Use "Join in Zoom App"

Once the meeting screen loads, you'll see a Join in Zoom App button at the top of the screen. Tapping it opens the appointment directly in the Zoom mobile or desktop app. 

You can switch over at any time during the appointment, and your provider will see you rejoin.

Troubleshooting

"It's not time for this appointment yet."

You're early. The join button activates 5 minutes before your scheduled start time. Refresh the page when it's time.

"Sorry, this appointment is no longer available."

The appointment may have been canceled, or the join window has closed (more than 60 minutes after the start time). Log into your account to check your scheduled appointments or reach out to support.

The video or audio isn't working

  1. Make sure camera and microphone permissions are granted to your browser (or to the Zoom app).
  2. If you're on Wi-Fi and seeing freezes, try switching to a different network or to cellular data.
  3. Tap Join in Zoom App to open the appointment in the Zoom app instead.

The meeting won't load at all

  1. Refresh the page.
  2. Tap Join in Zoom App to bypass the browser and join through Zoom directly.
  3. If you still can't get in, contact support so we can let your provider know.

I closed the window by accident

Reopen the join link from your reminder, or log into your Form Health account and tap Join Zoom on the appointment. You can rejoin as long as you're within the appointment window.
