How do I join my appointments?
You can join your appointment in two ways:
Through the Form Health mobile app
- At your appointment time, you can log into the Form app. You'll see a pop-up that prompts you to join the appointment through Zoom. The link to join will appear 5 minutes before your appointment, so you are encouraged to log in 5 minutes early in case you experience any issues.
Through the email reminders
All reminder emails include a link to join your appointment through Zoom. If you prefer to join from your computer or are unable to join through the Form app, you can use the link in the reminder emails to join your visit.
You do not need to have Zoom downloaded on your phone or computer to join your appointments. Once you click the Zoom link on either your phone or computer, you will have the option to "Join from your browser". This will open the Zoom call without downloading the app. If you prefer to have the app downloaded, you can download the "Zoom Workplace" app from your phone's app store.
