Why does Form Health ask if I have severe kidney disease, or on dialysis?

If you have advanced kidney disease or are getting dialysis for kidney failure we are unable to enroll you in our virtual clinic. This is because so many medications we prescribe are not safe for people with severe kidney disease or on dialysis. However, if you have other kinds of kidney problems or mild kidney disease, you might still be able to join and benefit from our science-based weight care and medical expertise. Please call Form Health and talk to a Care Advisor to learn more, and they will be able to help you determine whether you are able to enroll in our program.
