Why does Form Health ask about specific medical conditions in the enrollment process?
When you sign up for Form Health, we ask about your active health problems. Learning more about your health helps us ensure that our patients can safely get weight care through our telehealth clinic. First, certain health issues mean that it is not recommended to start a weight loss journey; examples of this are pregnancy, or active cancer. Other health issues make use of weight loss medications less safe, or contra-indicated. And finally, for people with highly complex medical conditions, for example after organ transplantation, they are better served by a coordinated in-person treatment team than by a telehealth program.
