Why does Form Health ask if I have had an organ transplant?

In most cases people who have had an organ transplant are not good candidates for the Form Health virtual clinic. Although there is some nuance based on the type of transplant, the reason for it, and the specific medicine to stop organ rejection, in general organ transplant recipients  will be better served by the hospital system where their transplant teams are, so that their complex care needs can be seamlessly integrated and all medication interactions can be carefully assessed.
