We're excited to partner with you on your weight loss journey.
Before we can schedule your first visits with your Form Health Medical Clinician and Registered Dietitian, you need to complete the onboarding tasks listed below. You can complete these in the Form Health app, or in the MyForm portal: my.formhealth.io

Once you download our app, you'll be prompted to upload your government-issued photo ID (like a driver's license or passport), your insurance card, and your prescription benefits card.
Note: Prescription benefit information might be on your primary insurance card. If you see “RX Bin”, “RX PCN” or “RX Group” on your primary insurance then you don’t need to upload a separate prescription benefit card.
I've completed my Onboarding Tasks, what should I expect next?
Now that you've completed your tasks and downloaded the app, make sure you have notifications enabled so you'll be notified when you receive a message from your care team.
Our enrollment team works behind the scenes to process your paperwork, collect your medical records from your primary care provider, and pair you with a care team that is licensed in your state and in-network with your insurance. Once we receive your medical records, you’ll be able to schedule with your care team! Your Enrollment Specialist and Patient Care Coordinator will be in touch with you through the Support Channel in the Form app.
When scheduling, please block off an hour for your initial appointments to ensure enough time, and make sure you'll be in your home state as required by provider licensing laws.
What should I do while I wait for my appointment?
If there is some lead time to your first appointment, feel free to start tracking your meals using our food log feature, or review the resources shared with you in the app.
