Navigating the Form Health App

For instructions on how to log into the Form Health app, please review this article.

When you log into the Form app, you'll see the homepage. The homepage includes:

  • Scheduling
  • Weight data
  • Food log
  • Resources

Scheduling

Appointments will be at the top of the page. If you don't have an appointment scheduled with one of your care team members, click "Schedule" next to your care team member's name.

Review this article to learn how to add an appointment to your personal calendar.

You can also reschedule an appointment by clicking "Modify", and confirm your appointment by clicking "Confirm". You won't be able to reschedule your appointment if it starts within 24 hours due to short notice, so you'll need to message your Care Coordinator to help. We kindly request at least 48 hours notice to cancel or reschedule your appointment.


Viewing your Weight History

Keep track of your weight change in the Form app by clicking "Show Weight History". This will expand your weight graph and weight change data. The graph will automatically update when you step on the Wifi connected scale we send you during onboarding. You can also add weight manually if you haven't received your scale yet.


Uploading a Food Log Entry

Keep track of your eating habits with the Food Log feature in the Form app. Simply snap a picture of what you're having (or upload a picture from your camera roll) and your dietitian will review!

Resources

Your dietitian, medical clinician, and care coordinator will share a variety of educational resources in the Form app. You can review them in the app or our MyForm portal online. If you're reading the resources on the app, you can pinch the screen to zoom in for easy reading.

In-app Chat Messaging

You can chat with your care team outside of appointments if you have any questions or concerns. To get to your chat channels, click the chatbox icon at the bottom of the screen. You'll see three chat channels (once your care team is assigned).

  1. Support
  2. Medical
  3. Dietitian

Support

You can message the support channel with questions regarding scheduling, billing, prior authorizations, medication refills, or anything else that comes to mind. If your Care Coordinator isn't able to answer your question, they will direct it to the right place. You'll also receive appointment reminders in the support channel.

Medical

Your medical clinician will be in the medical channel. You can message your medical clinician with questions about administering a medication or any medical questions. For more administrative questions, please message the Support channel, as they will likely see your questions sooner and be able to help.

Dietitian

Your dietitian will check in throughout the month to see how you're doing between appointments. Feel free to message your dietitian with nutrition questions or if you are needing a boost of motivation or support.
